Preparticipation Physicals

Parents,

Please be advised that incoming 7^th, 9^th and 11^th grade students that plan on participating in sports, cheerleading or band, must have a physical BEFORE practices of those activities begin. The physical form can be downloaded below.

The physical can be done at any time during the summer at our local clinic, Alto Family Medical Clinic.  The cost is $20.  Please let them know at the clinic that your son or daughter is a student of Alto.
