What Makes A Meal

 

Five components (Grain, Meat/Meat Alternate, Fruits, Vegetables and Dairy) must be offered to a student at lunch. The student must take 3 of the components for the meal to qualify for a reimbursable meal and one of the components must be a fruit or vegetable. Four items (Grain, Fruits or Vegetable and Dairy) must be offered to a student at breakfast. The student must take 3 items for the meal to qualify for a reimbursable meal and one of the components must be a fruit or vegetable.

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
